Base Salary: Depending on Experience
Utilizes a variety of treatment approaches to work with a diverse population. Provides therapeutic and case management services, specifically discharge planning and aftercare of patient hospitalization. This case manager will primarily work with individual who are struggling with addiction and substance abuse. It will involve outreach, assisting with transportation, connecting to resources, and other therapeutic services.
Bachelor’s degree in human services or related required. Previous experience with substance abuse and mental health-related case management and treatment. Minimum of two (2) years human service related experience preferred. Current valid Colorado Driver’s License required. Certified Addiction Counselor I or II is strongly preferred. Extra pay for Bilingual (English/Spanish).
