This Community Health Worker will have the opportunity to work within an integrated medical team, providing support and navigation assistance to individuals with high risk health issues. This position works with a designated clinical care team to provide comprehensive case management, home visits, health promotion to patients and serves as a health promotion instructor for designated class curriculum as well as periodic staffing support for outreach events.
Bachelor’s degree (BA/BS) in social service, health education, or public health preferred. Continuing education, training and/or two (2) years’ experience in healthcare may be considered in lieu of degree. Experience working with community organizations as an advocate for high-need individuals preferred. Case management tracking and documentation experiences or the ability to learn required. Ability to type, navigate an electronic health record are necessary skills. Bilingual preferred.
